Q: Can I have a support person or advocate with me?
A: Yes, we can involve a support person (for example, a family member) that you have chosen to advocate on your behalf. Counselling sessions are usually conducted in private but with your permission, your support person can be provided with relevant information at the end of the session to help you implement your treatment goals.
Q: What personal information do you collect about me?
A: B OK Psychology may collect personal information where this is required for the work we undertake. We will only collect personal information by lawful and fair means and primarily directly from you (for example, through our interaction with you or your use of our website). Wherever practicable any personal information is de-identified. Please refer to our Privacy Policy for the Management of Personal Information for more information.
Q: How is my personal information stored and who can see my information?
A: When you register as a patient of our practice, you provide consent for our psychologist and practice staff to access and use your personal information so they can provide you with the best possible care. Only staff who need to see your personal information will have access to it. If we need to use your information for anything else, we will seek additional consent from you to do this. Please refer to our Consent Policy for more information about this.
B OK Psychology takes all reasonable steps to keep your personal information secure, safe, and protected from misuse, interference, loss, or unauthorized access. Your information is either stored on secure servers (electronic records) using a health management system called Halaxy, or is protected in controlled facilities (hard copies of records are kept in a locked filing cabinet at our office and they are only accessible to the psychologist). B OK Psychology will keep your personal information as long as we are legally obliged to keep it, or until the need for the personal information has passed. At that point it will be destroyed, deleted or de-identified.
